What is a Vial of LIFE?
In medical emergencies, trying to think clearly can be difficult. A Vial of LIFE (Lifesaving Information for Emergencies) helps you provide medical information in advance that can be used by emergency personnel in case of an emergency. Emergency Medical Technicians will look for the Vial of LIFE. on your refrigerator and can pass this information on to the Emergency Room staff.
How to use the Vial of LIFE
Please do not send this form back to the company. It is for you to complete. Please make copies of your insurance cards and any Living Wills or Do Not Resuscitate instructions. Put these documents with your Vial of LIFE on top into the enclosed envelope with magnets on the back and attach to your refrigerator door in a visible location.Then, put the Vial of LIFE sticker near your front door or lockbox to alert emergency response personnel to look on your refrigerator for your medical information.
